How to Fill Out the Grade 3 SF9 Correctly

Step 1: Encode Learner Information

Start with the basic details — full name (in ALL CAPS, Last Name first), LRN, birthdate, and section. Double-check the LRN against your SF1 (School Register) to avoid mismatches.

Step 2: Transfer Grades from Class Record

Enter the final grade for each learning area per term. Grades are expressed as whole numbers from 60 to 100. Do not encode grades below 60 — the minimum passing mark is 60.

Step 3: Rate Core Values

Use your observation records, conduct reports, or behavior logs to assign ratings for each observable behavior under the four core values. Be consistent across all learners.

Step 4: Encode Attendance Data

Pull attendance data from your SF2 (Daily Attendance Record). Make sure the total number of school days per term is consistent across all learners in your class.

Step 5: Compute and Verify the General Average

The automated template will compute the General Average for you, but always double-check. The general average is computed by averaging all learning area grades, then rounding to the nearest whole number.

Step 6: Print and Have It Signed

Before distributing, have the form reviewed and signed by your School Principal. After card-giving, collect the parent/guardian signature and keep a record.
