What Is the DepEd SF9 (School Form 9)?
Before we dive in, let’s do a quick refresher — especially for new teachers or those new to the digital forms system.
School Form 9 (SF9), officially known as the Learner’s Progress Report Card, is one of the most important school documents you’ll handle as a class adviser. It is the official DepEd form used to report a student’s:
- Academic performance per learning area (grades from 60–100)
- Core Values ratings (Maka-Diyos, Makatao, Makakalikasan, Makabansa)
- Attendance record (days present and absent per month)
- Remarks on promotion, retention, or transfer
The SF9 is rooted in DepEd Order No. 4, s. 2014, which adopted the modified school forms for all public elementary and secondary schools. It is also guided by DO 11, s. 2018, which provides guidelines on the preparation and checking of school forms.
In short: this is the form your learners’ parents and guardians receive to know how their child is doing in school. Accuracy matters — a lot.
What's New for SY 2026–2027: The Three-Term Format
This school year brings a significant change that directly affects how the SF9 is structured: DepEd has shifted from a four-quarter calendar to a three-term (trimestral) system beginning SY 2026–2027.
Here’s what that means for the SF9:

This is critical. Using an old four-quarter SF9 template for SY 2026–2027 could create reporting inconsistencies. Always use a template that matches the current three-term structure.

How to Use the SF9 Template: Step-by-Step
New to automated Excel templates? Don’t worry — here’s a simple guide to get you started:
- Download the file from the download button below and save it to your computer.
- Make a backup copy immediately — save one “master” copy you never edit, and work on a separate copy.
- Open the PPT file — enable editing if prompted.
- Encode your school information — school name, division, district, school year, and section.
- Encode your class list — full names, LRN numbers, sex, and other learner details.
- Enter grades per term — input the grade for each learning area per term carefully.
- Record attendance data — encode monthly attendance figures for each learner.
- Fill in Observed Values — rate each learner on the four Core Values based on actual observation.
- Review all computed fields — check that Final Ratings, averages, and remarks are correct.
- Print a test copy first — before printing all cards, print one and check the layout on actual paper.
- Distribute with confidence!
Data Privacy Reminder: A Non-Negotiable
This is worth emphasizing: the SF9 contains sensitive personal information about your learners — their names, LRN, grades, and behavioral assessments.
Under the Data Privacy Act of 2012 (RA 10173), schools and teachers are obligated to protect this information. A few reminders:
- Do not post completed or filled-out SF9 files in Facebook groups or public Google Drive folders.
- Share only the blank template, not the completed class list.
- Store completed SF9 files securely and restrict access.
- When distributing printed report cards, do so privately and directly to parents or guardians.
