What Is the PACE Form?
The PACE Form — short for Pupil’s Achievement in Curricular Experiences — is a cumulative record form used in the early grades of the Philippine elementary curriculum. It is a comprehensive document that summarizes a pupil’s academic performance, attendance, and overall development across all learning areas for a given school year.
Think of it as a snapshot of your learner’s progress — a running record that follows the child from one grade level to the next. Unlike the report card (Form 138), which goes home to parents, the PACE Form is an internal school document kept as part of the pupil’s permanent academic file.
Why the PACE Form Matters
“A well-accomplished PACE Form is more than a compliance requirement — it is a tool for understanding each child’s learning journey.”
Here’s why this form is important for you and your school:
- Ensures continuity of learning — The receiving teacher in the next grade level gets a full picture of the pupil’s background.
- Supports early intervention — Low performance areas recorded in the PACE Form help identify learners who need additional support.
- Serves as a legal document — It forms part of the pupil’s permanent record and may be referenced during disputes or transfer requests.
- Feeds into school-level data — Division offices use cumulative data to assess school performance and design remediation programs.
Who Uses the PACE Form for Grade 1?
The PACE Form for Grade 1 is primarily accomplished by the Grade 1 class adviser at the end of the school year (or upon pupil transfer). Here’s a quick breakdown of who touches this document:
Role | Responsibility |
Class Adviser | Fills out all sections; certifies the form |
Subject Teacher | Provides grades for their specific learning area |
School Head / Principal | Signs and approves the accomplished form |
Division Office | Receives copies for centralized records (in some divisions) |
What Are the Sections of the PACE Form for Grade 1?
Understanding each section before you start filling it out will save you a lot of time and prevent errors. Here’s a breakdown of the standard sections in the Grade 1 PACE Form:
- Learner Information
This is the header section where you encode:
- Full name of the pupil (Last Name, First Name, Middle Initial)
- Date of birth and age upon enrollment
- Sex
- LRN (Learner Reference Number)
- Name and address of the school
- Section and school year
Pro tip: Always double-check the LRN against the School Register (SF1) to avoid discrepancies.
- Academic Performance by Learning Area
This is the heart of the PACE Form. For Grade 1, you will record quarterly grades and the final grade for each learning area. Under the current MATATAG Curriculum framework, Grade 1 covers the following subjects:
- Filipino
- English
- Mathematics
- Araling Panlipunan (AP)
- Edukasyon sa Pagpapakatao (EsP)
- Mother Tongue (if applicable)
- Music, Arts, Physical Education, and Health (MAPEH)
Grades are recorded based on the standards-based grading system outlined in DepEd policy. The passing grade remains 75, and grades are reported on a scale of 60 to 100.
- Attendance Record
This section captures:
- Number of school days present
- Number of school days absent
- Number of school days tardy
This information is taken directly from your Daily Attendance Report (SF2) and your class record. Make sure these match exactly.
- Remarks and Observations
Here, the class adviser may note any significant observations about the learner — behavioral concerns, outstanding achievements, referrals to the guidance counselor, or participation in special programs (e.g., remedial reading, gifted learner programs).
- Certification
This final section contains the signature of the class adviser and the school principal, certifying that all information recorded is accurate and true.

How to Fill Out the PACE Form for Grade 1: Step-by-Step
Filling out the PACE Form doesn’t have to be stressful. Follow these steps and you’ll breeze through it:
Step 1: Gather your source documents. Before opening the form, have these ready:
- SF1 (School Register) for accurate learner information and LRN
- SF2 (Daily Attendance Report) for attendance data
- Your E-Class Record or grading sheets for all quarterly grades
- Form 138 (Report Card) to cross-check final grades
Step 2: Encode learner information first. Start with the header. Fill in the pupil’s full name exactly as it appears in the LRN database. Any discrepancy here can cause problems during school transfers.
Step 3: Enter grades per learning area and quarter. Transfer the quarterly grades from your class record. Double-check each entry — transposition errors (e.g., typing 78 instead of 87) are common and can be hard to catch after printing.
Step 4: Compute the final grade. The final grade in each learning area is the average of all quarterly grades. For Grade 1, this is typically the average of Quarters 1 through 4.
Step 5: Fill in the attendance data. Get your total days present, absent, and tardy from SF2. Some teachers also keep a monthly attendance tally in their class record — use whichever is more accurate.
Step 6: Write your remarks. Keep this section factual and professional. Avoid subjective language. A remark like “Learner consistently struggled with reading comprehension across all quarters” is more useful than “Slow learner.”
Step 7: Print, sign, and have the form certified. Once you’re satisfied, print the form and submit it for the principal’s signature. Some schools keep digital copies as well — check your school’s document management practice.

Frequently Asked Questions About the PACE Form
Q: Is the PACE Form required by DepEd? A: Yes. Cumulative records are part of the school’s required documents. The specific form used may vary by division, but the obligation to maintain cumulative pupil records is mandated by DepEd policy.
Q: Can I use a printed template instead of handwriting it? A: Absolutely. In fact, using an editable digital template is preferred because it reduces errors and produces a cleaner document for school files.
Q: What if a learner was absent for most of the year and has no grade in some areas? A: Indicate “INC” (Incomplete) or “N/A” in the grade column and note the reason in the remarks section. Consult your School Records Officer for specific guidance.
Q: Is the PACE Form the same across all grade levels? A: The format is similar, but the learning areas listed differ by grade level. Make sure you’re using the Grade 1-specific version for your Grade 1 classes.
Q: Do I need to submit the PACE Form to the Division Office? A: This depends on your division’s specific guidelines. In most schools, the PACE Form is kept in the school’s filing system. The SF10 is the form more commonly submitted to the Division Office.
