⚠️ IMPORTANT WARNING: So-called “electronic school forms” are being sold to public schools. DepEd has authorized neither the usage of such forms nor their sale to schools.
School forms generated using these unauthorized solutions are considered null and void and will not be considered for official transactions.
Official school forms can be downloaded for FREE exclusively from the Learner Information System (LIS). Report any incidents of unauthorized sales to the LIS/EBEIS Helpdesk and Planning Service.
Division Checking Committee (DCC): What to Expect
